Thank you for your referral.
On exam, our patient was found to have non-proliferative diabetic retinopathy with macular edema. Given the morbidity of macular edema, I plan to proceed with intravitreal anti-VEGF injections for which I routinely start with Avastin (Bevacizumab). I follow a treat-and-extend protocol in where injections are performed every 4 weeks until the eye fully responds, then extend the interval of injections until the return of the macular edema.
Our patient was also found to have visually significant cataracts that are affecting their activities of daily living. After discussing with them the risks, benefits, and alternatives to cataract surgery, we decided together to proceed with surgery to remove the cataract once their diabetic macular edema is under control.
