​Thank you for your referral.

On exam, our patient had no signs of diabetic eye disease (E11.9).  I explained to our patient the need for tight glucose control in helping prevent future diabetic ocular complications.  In addition, our patient was found to have increased cupping of their optic nerves, putting them at risk for glaucoma (H40.023).  Baseline glaucoma testing will be performed and I will follow them as a glaucoma suspect.

I will have them follow up with us in one year for a repeat dilated exam, sooner as needed.
